그랜드 라인을 돌파한 써니호에 탄 루피 일행은 갑작스레 원인불명의 고열로 쓰러진 나미를 위해 의사를 찾아간다. 다행스럽게 그들이 도착한 곳은 의료대국으로 알려진 "겨울섬 드럼왕국"이었다. 먼저 호위대장 달튼에게 의사를 소개해 달라고 부탁하지만, 이 나라에는 의사가 눈에 갇힌 산꼭대기 성에 사는 마녀 닥터 구레하 한 명뿐이라고 한다. 루피와 상디는 나미를 데리고 닥터 구레하를 만나러 눈이 쌓인 산꼭대기 성으로 향하는데..
